With the Brumbies given a final warning after 65', every ensuring offence within 5m of their goal line simply has to be a yellow card.
So when Daley goes over with a double penalty advantage two minutes later (i.e. the ref had a penalty advantage for slowing it down and a NEW penalty advantage for slowing it down one phase later), the TMO not only calls against Daley, but the ref just doesn't card the two offending players? Why not??
In fact, Horwill even goes and talks to him WHILST the TMO is making his decision and says, 'that's three in a row, it has to be yellow', and the ref agrees.
At the very, very, very least he has to card one of them. Did he just forget to?
The last time I saw someone forget to card someone was in the 2006 FIFA world cup, whereby Graham Poll simply forgot to give a Red at a second yellow. He retired immediately.
It's an A grade error.

To make matters worse, in the NEXT phase, 13 White rushes up offside putting pressure on Ioane and making him drop it and bomb a try. It's a penalty try AND a yellow card. (15 v 10)
