If you look at the video for that entry the rule was designed for defenders trying to sneak into an offside position. Moore could have been done for incorrect binding though.
I'd guess the fact sheet may have mentioned that the Gold Coast 7s kicked off the IRB 7s season and a journo re-wrote it to be year not realising it really isn't the same thing.

The timing of the warning is at the Refs discretion, the actions afterwards are not.

So the way I read that is after a warning, the next penalty given for the same offence is an automatic yellow, if another team member does it again it's a red.
